Joe Manganiello's Muscular Physique Took Years to Build
Manganiello's commitment to fitness helped define his appearance on screen, particularly after he landed the role of werewolf Alcide Herveaux in HBO's "True Blood."
In a 2014 interview with Men's Health UK, the actor recalled working in construction after his acting career slowed following his appearance in the 2002 film "Spider-Man."

He spent long hours moving sand and gravel in the Los Angeles heat before heading to the gym after work.
Describing how intensely he pushed himself, Manganiello said, "I didn't just train after work: I hit the gym like an animal. I pushed myself to the point where people around me were borderline frightened."

His training intensified when he joined "True Blood" in 2010. He worked with coach Ron Mathews for five months to build the muscular frame audiences came to recognize.
Preparing for "Magic Mike" required even more demanding workouts. Manganiello recalled performing thousands of push-ups, curls, and other exercises while following a strict diet.

Manganiello ended his message with encouragement for young people who struggled with physical fitness.
"This is for all of the kids out there who couldn't do one pull up or one dip into high school, it's not how you start the race, it's how you finish... oh, and you never finish," he wrote.
What fans could not see in that gym photo was the serious medical ordeal Manganiello had already survived.
Joe Manganiello's Hospital Photos Show the Toll of His Illness
On October 15, 2026, Manganiello shared three Instagram photos documenting the physical effects of a mysterious illness he had battled for years.
The first image shows him sitting upright in a hospital bed, wearing black-framed glasses and sporting a thick beard. His shoulders, arms, and chest appear noticeably thinner than in the muscular fitness photos fans had grown accustomed to seeing.
A blanket covers his lower body, while medical tubes and equipment are visible around him. He described how rapidly his weight changed following the operation.

"In the days following the amputation of my large intestine, my weight began to plummet. I would ultimately lose a total of 60 lbs during the two weeks following the surgery," he wrote.
Manganiello also noted that the top of his first ostomy bag was visible above the hospital sheets. The actor shared the photos under the title "BLOODLINES • Chapter 1 • In pictures," referring to his memoir "Bloodlines," released on October 13.
Two More Photos Show the Physical Effects of Manganiello's Health Battle
The second image focuses on Manganiello's abdomen following another operation. Surgical scars and marks are visible from the procedure, which involved removing his ostomy bag and reconnecting his digestive tract.
Manganiello identified the surgery as an anastomosis, a procedure that reconnects sections of the digestive system. The third photo shows another health problem he experienced during his illness.
In the close-up image, redness and irritated patches are visible across his forehead, cheeks, and nose. "An example of the multitude of skin conditions I was plagued with during the seven years I was sick," he wrote. He added that several skin conditions often affected him at the same time.

Joe Manganiello Recalls the Night He Feared He Would Die
In an October 2026 interview with People, the actor revealed that he had battled a mysterious autoimmune illness for seven of the previous ten years.
Despite undergoing numerous tests and trying different medications, doctors never reached a conclusive diagnosis.
Manganiello said treatments used for nerve pain, Crohn's disease, and colitis failed to relieve his symptoms. His condition became critical during a hospitalization in 2016.
In an excerpt from "Bloodlines" published by People, Manganiello recalled suffering severe abdominal pain while his then-wife, Sofia Vergara, slept nearby.

He described seeing his reflection during one particularly difficult night. "I was beyond gaunt, and my skin was translucent, like thin newspaper," he wrote.

During the operation, they discovered that his large intestine was severely damaged and close to rupturing. According to Manganiello, a rupture could have caused fatal sepsis.
While he was under anesthesia, doctors contacted Vergara to authorize the removal of his large intestine. She agreed to the life-saving procedure, which left him with an ostomy bag.
Manganiello woke up after the ten-hour operation to learn that he had survived but might need the bag permanently. The uncertainty frightened him, particularly after years of working to build his career and maintain his health.
Joe Manganiello Reveals Where His Health Stands After Seven Years
Surviving emergency surgery was only the beginning of Manganiello's health battle. According to People, the actor spent the next seven years moving in and out of hospitals as doctors struggled to identify the cause of his symptoms.
The years of uncertainty also affected his personal life, putting a strain on his marriage to Sofia Vergara before the couple divorced in 2023.
After years of unsuccessful treatments, Manganiello began exploring alternative approaches to healing. He came to believe that addressing his family's history of trauma played a role in his recovery.
His great-grandmother had lost nine children during the Armenian genocide, while his grandmother had suffered from lupus.
Manganiello also underwent another operation to reconnect his digestive tract and remove his ostomy bag. He has since reported being symptom-free and no longer taking medication.
After years of fearing for his life, the actor reflected on how much his outlook had changed. "For seven years I thought I was going to be dead the next week," he said. "The world has opened back up to me."
